Objective: In a metastatic stage prognosis of patients with soft tissue sarcoma (STS) remains poor with a median survival of 1-2 years under anthracycline-based chemotherapy as standard treatment. Local therapies (LT), mainly surgical resection and radiotherapy, are increasingly applied for patients with oligometastasis. The value of LT is insufficiently defined regarding the level of evidence. So far, only non-STS-specific phase II studies exist on this question. For patients with 1-5 metastases of different cancers a survival advantage was shown after addition of consolidative LT to a systemic therapy. Prognosis of patients with oligometastatic non-small cell lung and colorectal cancer could be improved by LT leading to its recommendation in the respective German S3 guidelines. Since there are no evidence-based standards for the use of LT in oligometastatic STS patients, the indication is determined situationally in multidisciplinary tumor boards based on criteria such as number, size, localization and resectability of metastatic lesions as well as disease progression and response to systemic therapy. The patients and STS subtypes who take the greatest advantage of LT have not been identified yet. To date, treatment algorithms for patients with oligometastatic STS are not established.
Evaluation of our data will hopefully lead to a considerable gain of knowledge about the poorly understood clinical picture of oligometastasis. Our goal is to establish evidence-based recommendations for LT in order to improve quality in the care of oligometastatic STS patients.

Methods: The present study is a retrospective, multicenter trial designed to evaluate the role of LT for metastatic STS. The project is funded by Bayerisches Zentrum für Krebsforschung (BZKF) and takes place in all 6 BZKF sites (LMU and TU Munich, University Clinics Erlangen, Regensburg, Würzburg and Augsburg). Included are all patients with metastatic STS who were recommended a metastasis-directed local ablative therapy in one of the BZKF
tumor boards in the years 2017-2021. The expected patient number to be enclosed is around 350. The primary objective is to evaluate efficacy in terms of progression-free survival (PFS), local-progression-free survival (LPFS) and time to next treatment (TNT) after local therapy. Secondary objectives include overall survival (OS) and toxicity. In cooperation with all disciplines involved in sarcoma therapy a comprehensive case report form has been developed. In the context of this project the entire course of disease of this patient cohort including detailed information about all local and systemic therapies is recorded. In parallel, the implementation of a common prospective registry also offering opportunities for translational research has already been started.

Results: At date of upcoming CTOS meeting in November 2023, retrospective data will be evaluated and results can be presented. However, the prospective part will still be in progress.

Conclusion: At date of upcoming CTOS meeting in November 2023, a conclusion about the use of local ablative therapy in oligometastatic soft tissue sarcoma will be drawn based on our results.
