(P 403) THE IMPACT OF PATIENT, TUMOR, AND TREATMENT FACTORS ON SURVIVAL AND METASTASIS OUTCOMES IN PATIENTS WITH MYXOID LIPOSARCOMA

Objective: Myxoid liposarcomas are rare soft tissue tumors which pose challenges to physicians attempting to develop treatment protocols or deliver prognostications to patients. Current literature on myxoid liposarcoma has noted a few prognostic factors such as tumor size and grade; however, optimal therapeutic strategies, including the role of chemoradiotherapy, remain debated. This retrospective study seeks to identify patient and tumor factors associated with disease-free, metastasis-free, and overall survival rates in patients with myxoid liposarcoma. Additionally, we evaluated the efficacy of different treatment modalities and their association with patient outcomes. By identifying factors that influence prognosis, this study may contribute to better patient education, informed decision-making, and the development of personalized treatment strategies to improve outcomes in patients affected by myxoid liposarcoma.

Methods: A retrospective cohort study was done using data from a large academic institution’s sarcoma archives. Patients diagnosed with myxoid liposarcoma between January 1, 1997, and June 24, 2021, were considered for this study. Patients over 18 years of age with histopathologic confirmation of myxoid liposarcoma and no known metastases at the time of presentation were identified for further review. Patient demographics, tumor characteristics, treatment(s) received, and outcomes were collected from electronic medical records. Statistical analyses were performed to assess the impact of various factors on disease-specific survival and metastasis-free rates at different time endpoints.

Results: Our cohort of 66 patients had a 15-year disease-specific survival rate of 73%. In addition, 56% of our cohort remained metastasis free at the same time endpoint. Demographic factors such as age, sex, race, body mass index (BMI), and smoking history did not demonstrate a significant association with survival outcomes. Tumor grade and volume also did not show a significant impact on survival rates. Patients who underwent primary tumor resection at our tertiary center had improved survival compared to those treated at outside hospitals. Notably, neoadjuvant chemotherapy and adjuvant radiotherapy demonstrated favorable outcomes compared to other treatment approaches. Moreover, patients who received combined chemoradiotherapy had significantly better outcomes compared to patients who received monotherapy.

Conclusion: This study contributes to the understanding of myxoid liposarcoma by identifying potential prognostic factors and treatment modalities that can enhance patient outcomes. The findings suggest that chemotherapy and adjuvant radiotherapy may improve survival rates in myxoid liposarcoma patients. However, further research is needed to validate these results and explore additional molecular markers and genetic abnormalities that may influence prognosis. Nonetheless, this study provides valuable evidence for the development of personalized treatment strategies to improve outcomes in myxoid liposarcoma.
